Health Care Excel Welcomes the Episcopal Church Home MedCare Pharmacy Team to the Patient Safety and Clinical Pharmacy Services Collaborative (PSPC).
The Episcopal Church Home, based in Louisville, Kentucky, is an interdenominational Christian community retirement and care facility. The facility is participating in the Patient Safety and Clinical Pharmacy Services Collaborative (PSPC) in partnership with MedCare Pharmacy and Health Care Excel,the Kentucky Medicare Quality Improvement Organization (QIO). The facility provides a full range of resident services including long-term Alzheimer’s Disease/memory care, short-term disability rehabilitation, personal care and independent living. The newly create team is monitoring high-riskpatients receiving care for diabetes, and/or anticoagulation therapy, and/or antipsychotic use. Kathy Schireman, Director of Clinical Services, expects great outcomes from participation in the collaborative.
Episcopal Church Home expectations align with the PSPC goals of integrating evidence-based clinical pharmacy services into the care and management of high-risk, high-cost, complex patients and the QIO’s goals of reducing potential adverse drug events (pADE) and adverse drug events (ADE). A Clinical Pharmacist from MedCare Pharmacy will be incorporated into the Episcopal Church Home care
management team, creating a multidisciplinary approach, providing clinical intervention. Megan Wesselman, Clingman Neighborhood Nurse Manager and lead for the PSPC/QIO drug safety team at Epsicopal Church Home, recently worked with the inter-professional team to develop and implement a policy and documentation form for anticoagulation therapy.
